These Terms of Use govern your use of bighammobility.com, operated by Bryan Bigham Physical Therapy, LLC, doing business as Bigham Mobility. By using this site you agree to them. If you do not agree, please do not use the site.
1. This website is not medical advice
Everything on this site is general education about physical therapy. It is not a diagnosis, not a treatment plan, and not a substitute for an in-person evaluation by a licensed clinician. Reading this site, submitting a form, or speaking with us by phone does not create a therapist-patient relationship. That relationship begins only when you and Bryan Bigham Physical Therapy, LLC sign a written agreement for services and an evaluation is performed.
If you are having a medical emergency, call 911. Do not use this website or email to report an urgent medical problem.
2. Licensure and scope of practice
Services are provided by Stephen Bryan Bigham, PT, DPT, MHA, licensed as a physical therapist by the Texas Board of Physical Therapy Examiners (license #1166380). Services are offered only to individuals physically located in the State of Texas at the time of service, whether in person or by telehealth. We do not provide care to residents of other states.
Texas permits evaluation and treatment by a licensed physical therapist without a physician referral, subject to the limits in the Texas Physical Therapy Practice Act. If your condition requires care outside the physical therapy scope of practice, or if you do not show measurable improvement within the period set by state law or rule, Bryan will tell you and will refer you to a physician or other appropriate provider.
3. Telehealth
Telehealth visits are conducted by live video. They are appropriate for follow-up, coaching, home program updates, and progress checks, and they are not appropriate for every condition. Video care has limits, including the inability to perform hands-on assessment and treatment and the possibility of technology failure. You must be located in Texas at the time of a telehealth visit. Bryan will tell you when an in-person visit is the better choice.
4. Payment, self-pay status, and insurance
Bigham Mobility is a self-pay practice. Prices shown on this site are current at the time of posting and may change. The price in your signed agreement is the price you pay. Payment is due at the time of service unless we agree otherwise in writing.
We are not currently contracted with any commercial insurance plan. On request, we can provide an itemized statement you may submit to your plan, but we make no promise of reimbursement, and any deductible, out-of-network benefit, or denial is between you and your insurer.
Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of the expected cost of your care before services begin. Ask for one at any time and we will provide it in writing.
5. Cancellation
In-home visits require travel and a reserved block of time. Please give at least 24 hours notice to cancel or reschedule. Cancellations with less notice, and missed visits, may be charged in full. Specific terms are set out in your service agreement.
6. No guarantee of results
Physical therapy outcomes depend on your condition, your history, and your participation in your home program. Nothing on this site is a promise or guarantee of any particular result. Any patient story or example is one person’s experience and is not a prediction of yours.
7. Your use of the site
You agree not to use this site unlawfully, not to attempt to gain unauthorized access to it, not to submit false information, and not to use its forms to send spam, malware, or commercial solicitation. We may block or remove submissions that violate this section.
8. Intellectual property
The text, layout, photographs, and logos on this site are owned by Bryan Bigham Physical Therapy, LLC or used with permission. You may view and print pages for your own personal use. You may not republish, copy, or use them commercially without written permission.
9. Links to other sites
Links to third-party sites are provided for convenience. We do not control them and do not endorse their content.
10. Disclaimer and limitation of liability
This website is provided on an “as is” basis without warranties of any kind, express or implied, including any warranty of accuracy, availability, or fitness for a particular purpose. To the fullest extent permitted by Texas law, Bryan Bigham Physical Therapy, LLC is not liable for any indirect, incidental, or consequential damages arising from your use of this website or from reliance on its content. Nothing in this section limits liability for professional services actually rendered, which are governed by your service agreement and by Texas law.
11. Governing law
These terms are governed by the laws of the State of Texas. Venue for any dispute lies in Denton County, Texas.
12. Changes
We may update these terms. The version posted here, with its effective date, is the version that applies.
